They have to add all the cards that come out to their hand.

UNO Attack Replacement Cards

The replacement cards in the Uno attack are the cards used to replace the damaged or missing cards. However, you can use the blank cards as replacement cards, and to do this, you need to figure out which card is damaged or missing. When you figure it out, just take a blank card, write that missing card’s name or role, and add it to the game. This card now functions the same as the original cards, and you may not need to buy new cards as a standard deck involves four blank cards.
